The MVZ Entity Relationship Diagram (ERD)
The Data Model

Development of a new database management system for the MVZ has included refinements to the original data model posted on this web site in 1996. We will continue to provide the most current ER version of the model in its entirety in two formats, viewable on-line or downloadable and printable as a 5 X 3 page matrix on letter-size paper. Interested persons should continue to refer to the 1996 logical and conceptual models for background information. DDL's for implementing the model on most DBMS's are available upon request.
